Fruit machine coding

Though I am not much of a coder myself, I was wondering if anyone would know this. Looking at all the emptiers/tricks of the past, how easy /difficult is it to make a coding error that allows this to happen. I remember say the DK cancel one being a debug routine being left in the program. Just curious thats all

Not hard to do, just not easy to hide ........wasn't it barcrest who got rid of a few guys for this, and Astra too!
